Dexterity sporadic care is designed for patients in the need of infrequent medical assistance.. Sporadic care enables patients to receive the highest quality healthcare only when assistance is required, eliminating the cost of full-time, around-the-clock care. Trained Medical Care

Regular scheduled visits by a Registered Nurse (RN) or Licensed Practical or Vocational Nurse (LPN/LVN), to make sure that each patient gets appropriate and professional care while attending to all clinical necessities.

Certified Home Health Aides

Certified Home Health Aides (CHHAs) provide assistance with activities of daily living and personal care with the supervision of an RN.

Companion Care

Experienced and trained companions are available in a short term or on request basis to help patients with various sporadic activities under the supervision of Dexterity Care appointed RN.

Home Care Management

In cooperation with the patient's Physician, Dexterity Care medical team provides services for individuals with severe or chronic illness. Our team works with patients to provide individual care management plans that meet their needs. Dexterity Home Care Management Program is complemented by clinical pathways and disease management programs which help to reduce long-term complications of conditions such as heart disease, stroke, cancer, and diabetes.

Therapy Services

Dexterity's therapists provide a personalized rehabilitation program based on each patient's needs. Our staff includes Physical Therapists, Respiratory Therapist, Occupational Therapists, Speech Language Pathologists, and Medical Social Workers.

Rehabilitation

Dexterity rehabilitation therapists provide specific treatments and instructions on home safety, balance, orthopedic, and neuro-rehabilitation, with the objective of returning patient's independence and self-control. Our therapists design specific rehabilitation programs to meet the needs of each individual patient.

More often, for certain reasons, a caregiver may need to leave their loved one alone, but feel uncomfortable leaving them alone without a certified medical caregiver. Our knowledgeable and experienced staff conducts in-depth interviews with the patient, family and/or relatives to determine the type and level of care required, enabling patients to receive the care they need from the comfort of their home.

All members of Dexterity Sporadic care team are licensed and trained. We understand that each individual's healthcare requirements are unique and we put all our efforts to satisfy these needs. That is why we make sure that we assign not only the right caregiver to our patient but also the most compatible one.Sporadic care staff is available on a temporary or long-term basis, depending on the patient and caregiver requests. We start by sending a nurse or therapist to the home for an initial consultation and evaluation with you and your Physician to ensure that we develop a detailed and effective plan of care for the patient.
